Bridges Wound Care

A chronic would (defined as a wound that does not how significant progress in healing within 30 days) often indicates underlying medical issues such as vascular problems, heart conditions, or diabetes. At Bridges Palliative care, we provide comprehensive care for all types of chronic wounds. We evaluate causes for non-healing wounds and create a care plan for each individual. Whether your wound requires sharp debridement, skin substitutes, or both, we are here to help.

We bring the wound center to you! Our customized electronic health record and database is able to integrate seamless documentation incorporating wound measurements through an assortment of tracking tools to identify wound healing progress and gap analysis. This modality allows for administrative and clinical wound education.

Accreditation

Our services will assist in the requirement and application process for your facility to achieve accreditation and certification of a quality wound care management program. This center of wound management certification is accredited through recognition of qualified, competent care providers and staff, standardized care delivery process through clinical practice guidelines and performance measurements to monitor, analyze and used to improve patient outcomes.
